If you register as a Dependent under NHI (pibuyangja), the insurance premium is 0 KRW.
Family members of a Workplace subscriber (a person who works at a company) can be registered as a Dependent under NHI (pibuyangja).

Your spouse and children under 19 can register right away without waiting 6 months. Parents need to wait 6 months (except Permanent Residence/Marriage).
Spouse registration: the easiest
Your spouse can be registered most easily as a Dependent under NHI (pibuyangja).
Marriage (Marriage) spouse:
→ You can register right after entering the country (6-month waiting period exempt)
→ Additional insurance fee: 0 KRW
→ There is also an automatic processing system linked to resident registration
❓ What if your spouse is also an employee?
If you are a dual-income couple (both spouses are employees), each person is enrolled in their own workplace health insurance. In this case, you do not need to register as a Dependent under NHI (pibuyangja) — the insurance does not apply twice. Children can be registered as a Dependent under NHI (pibuyangja) of either parent.
Spouse with another visa:
→ You can also register right away (because of the 2024 change, spouses are exempt from the 6-month waiting period)
→ But you must have Alien Registration
Required documents for family registration (prepare these first!):

📋 Documents proving family relationship (marriage certificate, birth certificate, etc.) — Korean translation + notarization required

📋 Documents confirming cohabitation (if needed)
You need an apostille (128 countries) or embassy certification. In Vietnam, Cambodia, Nepal, Myanmar, and some other countries, apostille is not available, so please get embassy certification.

Child registration depends on the situation.
Child born in Korea:
→ From the date of birth, they are automatically included in the parents’ health insurance
→ Child of a Workplace subscriber: automatically registered as a Dependent under NHI (pibuyangja)
→ Child of a Resident subscriber: automatically included in the household
→ It is processed when you complete birth registration + Alien Registration
Children born overseas and entered the country:
→ Under 19: can register right after entry (6-month waiting period exempt)
→ 19 and older: 6-month waiting period applies
Adult children aged 19 or older:
→ To keep dependent status under NHI (pibuyangja), they must meet the income and property requirements
→ If they get a job, they become a Workplace subscriber, so they lose dependent status under NHI (pibuyangja)
Inviting your parents: there is a 6-month waiting period.
When you invite your parents to Korea, health insurance is a problem.
Changes in April 2024:
→ Foreign parents can register as Dependent under NHI (pibuyangja) only after living in Korea for at least 6 months
→ Before, they could register right away, but this was made stricter to prevent abuse of medical tourism
→ However, parents with Permanent Residence(Permanent Residence) or Marriage(Marriage) visas can register immediately
How to deal with the 6-month waiting period:
→ Before leaving your country, sign up for travel insurance / short-term international insurance
→ In an emergency, you can use the emergency room (regardless of Status of residence)
→ After 6 months, when you register as Dependent under NHI (pibuyangja), extra insurance fee is 0 KRW
Parents' income and asset conditions:
→ Annual income under 20,000,000 KRW
→ Property tax assessment standard under 540M KRW
→ If there is no income in Korea, most people qualify
Family relationship documents must be issued within 9 months from the issue date. Old documents need to be reissued.
Resident subscriber rules are different.
Unlike Workplace subscriber, family insurance for Resident subscriber has more restrictions.
Family members who can be combined in one household:
→ Only a spouse + children under 19 can be combined into the same household
→ Parents, adult children, and siblings are separate households = insurance premiums are paid separately
Important: In Korea, parents can also be included in the same household, but foreigners cannot.
Household combined calculation notice:
→ It is not automatic. You need to apply to NHIS yourself
→ Alien Registration Card (ARC) + family relationship certificate documents are needed
Premium impact:
→ Resident subscriber premiums are based on income, property, and cars
→ If your household is combined, the whole family’s income and property are added together, so the premium may go up
→ Check with NHIS whether combining is better or paying separately is better
It is not automatic. You need to apply directly to NHIS, and first check whether combining is better for you.
