What happens to health insurance when your visa expires? A summary including how to prevent a gap in coverage.
Insurance automatically ends the day after your visa expires. Unpaid insurance premiums directly affect visa extension. Even without registration, emergency treatment is possible.
Visa expiration = insurance automatically ends
Korea’s National Health Insurance (NHIS) is connected to your Status of residence.
When your visa (period of stay) expires, your health insurance eligibility automatically ends the next day. There is no grace period.
You do not need to report it separately. Your immigration record is automatically sent to the National Health Insurance Service.
It is the same when you leave the country:
→ If you stay overseas for more than 1 month, you lose your eligibility
→ The insurance premium is calculated by day based on the date eligibility is lost
→ You can get back any overpaid amount
Before changing your visa, be sure to call NHIS(National Health Insurance Service (NHIS)) and check how your insurance eligibility will change.
If your visa changes, your insurance changes too.
If your visa type changes, your insurance type may also change.
You can apply starting 4 months before it expires. If you do it right before, there may be a gap while it is being reviewed.
If you do not pay the insurance premium, it can affect your visa.
Overdue health insurance premiums directly affect visa extension.
Arrears check system (from August 2019):
→ When you apply to extend your visa, the immigration officer checks whether you have unpaid health insurance premiums
→ If you have arrears, they ask you to pay
→ If you pay everything, normal extension is possible (2~5 years)
→ If unpaid, only a limited extension of up to 6 months is possible
Suspension of NHI-covered service (geupyeo) (revised and effective on 2025.4.23):
→ Before: Foreign nationals had NHI-covered service (geupyeo) suspended immediately after just 1 unpaid contribution (there was no notice)
→ Now: If contributions are unpaid 3 times or more, NHI-covered service (geupyeo) is limited (improved by a Constitutional Court decision)
→ Koreans: limited after 6 or more unpaid contributions + prior notice
Foreigners are still at a disadvantage compared to Koreans, but it has gotten much better than before.
If you have unpaid payments, you can only extend the limit for up to 6 months. Please make sure to check whether you have unpaid amounts in the The건강보험 (official NHIS app) (official NHIS app) app.
Even if you are staying illegally, you can still get emergency treatment.
If your visa has expired, you cannot enroll in health insurance. But that does not mean you cannot get treatment at all when you are sick.
Protected by the Emergency Medical Service Act:
→ Emergency patients can get treatment regardless of their Status of residence
→ If a hospital refuses emergency treatment, it is illegal
Foreigner medical support program (Ministry of Health and Welfare):
→ For foreign workers who cannot receive health insurance benefits
→ Undocumented foreigners are also included
→ Support mainly for hospitalization and surgery (outpatient care is excluded in principle)
→ 90% of medical costs covered, limit 5,000,000 KRW per case
→ Children under 18 also get support for outpatient care
But this is the last safety net. The most important thing is not to miss your visa extension.
Some changes (Non-professional Worker→Miscellaneous, etc.) may cause you to lose your insurance eligibility itself. Please make sure to check before changing.
If you come back to Korea, what happens to your insurance?
If you come back to Korea after leaving the country, it depends on your situation.
Re-entry within 6 months:
→ If your stay period is still valid and you paid the insurance premiums while abroad, your eligibility is restored immediately upon entry
→ If there are unpaid amounts, it will be restored after full payment
Re-entry after more than 6 months (or a new visa):
→ It is processed based on a new Alien Registration
→ The 6-month waiting period may apply again
→ But Student (Student), Non-professional Worker, Permanent Residence (Permanent Residence), and Marriage (Marriage) can enroll right away
What changes from July 2025:
→ While you are abroad, your insurance benefits are suspended (benefit suspension = your insurance stays active, but you cannot get support for hospital costs)
→ Before, after re-entering Korea, you had to report directly to NHIS for the benefit suspension to be lifted
→ From July, if you re-enter Korea after staying abroad for 3 months or more, the benefit suspension is automatically lifted without a separate report
→ This works by automatically sending entry/exit data to the Corporation the day after entry
Medical expense reimbursement insurance (silson insurance) is not directly related to a visa.
Medical expense reimbursement insurance (silson insurance) is private insurance, so the contract stays active no matter your visa status. But if you leave the country, you cannot use hospitals in Korea, so the coverage has no real benefit, and you only keep paying the premium.
If you can prove that you stayed overseas for more than 3 months, you can stop paying insurance premiums.
If you resume payments after re-entering the country, your coverage will continue.
If you don’t plan to come back, it’s reasonable to cancel.
Before canceling, be sure to check whether there is any unclaimed insurance money (3-year statute of limitations)
If you plan to re-enter the country, you can also keep it after stopping payments.
